Responsibilities- Provide direct patient care to residents in our long-term care facility, including administering medications, wound care, and other treatments as prescribed
- Monitor and report resident conditions, changes, and concerns to the nursing staff and physicians
- Assist with daily living activities such as bathing, dressing, and grooming as needed
- Participate in developing and implementing individualized care plans for each resident
- Collaborate with other healthcare professionals to ensure comprehensive care
- Maintain accurate and timely records of resident care and treatments
- Current LPN license in the State of Pennsylvania
- High school diploma or equivalent required
- 1+ year of experience as an LPN in a long-term care setting preferred
